NHTD & TBI Waiver — stay home, not in a facility.

New York's Nursing Home Transition & Diversion (NHTD) and Traumatic Brain Injury (TBI) Medicaid waivers let people who'd otherwise need a nursing home live in their own home with support. Amelia handles the entire application and ongoing coordination.

What These Waivers Cover

A real alternative to a nursing home.

The NHTD waiver serves seniors and adults with physical disabilities who would otherwise need nursing-home placement. The TBI waiver serves people aged 18–64 with a documented traumatic brain injury (including post-stroke survivors). Both pay for in-home care, modifications, transportation, and supportive services so the person can stay home.

Services covered

  • Service Coordination — a dedicated case manager (usually Amelia)
  • Community Integration Counseling and Independent Living Skills training
  • Home and environmental modifications (grab bars, ramps, widened doorways)
  • Respite for family caregivers
  • Personal Emergency Response System and assistive technology

The person must be Medicaid-eligible (or able to become eligible) and meet nursing-home level of care on a clinical assessment. We can usually tell you in one call whether you're likely to qualify.
